The North Lincolnshire Health & Care Partnership is inviting residents, carers, families, and health and social care professionals to take part in a short survey to help improve community equipment and digital services across the area.
The survey forms part of the Independence First, Equipped for Life project and is seeking feedback from people who have received specialist equipment on loan from the Scunthorpe Equipment Stores, as well as those who support or refer people into these services.
Your experiences and insights will help shape future services, ensuring they better meet the needs of people across North Lincolnshire and improve outcomes for local residents.
The survey takes around 5 minutes to complete and is open until Friday 8 August 2026.
